Give the findings
somewhere to go.
Build a report covering every form, with the most serious findings first. See its status, the reasons it was flagged, stored entry counts, data destinations, and the pages where it appears.
THE PRO ADD-ON FOR FORM MARSHAL
Know what your WordPress forms collect. Give your team the reports, network rules, and updates to do something about it.
Built on the free Form Marshal plugin. Made for teams with more to keep track of.
FORM PRIVACY REVIEW
Illustrative report. Fictional example data.
WORKS WITH THE FORMS YOU ALREADY USE
FROM FINDING TO FOLLOW-THROUGH
The free plugin finds forms that may collect health or personal information, explains why, and can block them. Pro helps you bring those findings to the people who need them.
Share a review with your privacy team. Keep minimum rules in place across a WordPress network. Get a daily summary without opening every site's dashboard.
WHAT PRO ADDS
For site owners, agencies, and teams responsible for the forms behind the website.
Build a report covering every form, with the most serious findings first. See its status, the reasons it was flagged, stored entry counts, data destinations, and the pages where it appears.
See each site's counts in WordPress Network Admin. Set minimum enforcement, maximum retention, and entry sampling rules that individual sites cannot loosen.
For WordPress multisite networks.
Send your team a daily email with finding counts and a list of health-information forms. Send only when findings change, or choose a summary every day.
Send form events to a URL you choose. Add a shared secret to sign each message so your receiving system can verify it. Messages carry event details, never form answers.
Connect your monitoring tool to a protected health check. Track health-information forms, blocked forms, exceptions expiring within 14 days, and the last scan time.
Download the audit log as a CSV file for your records or review process. The file streams to your browser, and the export itself is recorded in the log.
BUILT ON FORM MARSHAL
The free plugin shows which forms need attention and why. Pro turns those findings into reports, summaries, and network oversight.
Explore the free pluginActual Form Marshal dashboard with fictional demonstration forms. Pro installs alongside it.
USEFUL RECORDS. FEWER SENSITIVE DETAILS.
Pro reports are built from findings, not submitted form answers. Event messages also leave out entry values.
Form Marshal helps you apply your policies. It does not certify HIPAA compliance or replace a privacy review.

GETTING STARTED
Activate the free plugin from WordPress.org and a supported form plugin.
Upload and activate your Pro ZIP. For multisite, network-activate it.
Use Activate License on the Plugins screen. Then set up reports, notifications, or network rules.
Yes. Pro is a separate add-on, not a replacement. It needs Form Marshal 0.5.0 or newer, active and running, with a supported form plugin. Both require WordPress 6.4 or newer and PHP 7.4 or newer.
No. Form Marshal flags likely problems and gives you tools to apply your organization's policies. Neither Free nor Pro certifies compliance or replaces a security and privacy review.
Yes, within your license's site allowance. The network dashboard and shared minimum rules work within a WordPress multisite network. They do not create a central dashboard for unrelated WordPress installations.
A super admin can activate a license across a network. Each site counts as an activation, so choose a license with enough sites for the network.
Pro's features keep working. Updates and support stop until you renew. The free plugin continues to work independently.
Pro uses Freemius for licensing and updates. A licensed site shares its address, software versions, and Pro activation status. Activation may also send the administrator's name and email. Configured digests send summaries to your chosen recipients, and event messages go to your chosen URL. Reports and event messages contain no submitted entry values. Read the privacy details.
Use the Form Marshal support repository on GitHub for questions, bugs, and feature requests. Issues are public: do not post form answers, patient information, license keys, or passwords. Report security issues privately through GitHub's private vulnerability reporting.
